One poignant message came from a young woman who shared her struggle with a recent cancer diagnosis. She expressed her fear of losing her ability to conceive due to the impending chemotherapy treatments. Stories like hers are not uncommon. In our clinics and across social media, we often encounter patients seeking guidance on how to safeguard their reproductive futures. This has led to an increasing awareness and demand for fertility preservation techniques.
Fertility preservation involves the proactive cryopreservation (process of freezing to preserve for future use) of gametes (eggs or sperm), embryos, or reproductive organ tissue for future reproductive use. Advances in assisted reproductive technologies (ART) have made it possible for individuals at risk of losing their reproductive potential to conceive later in life.
One of the most established recommendations for fertility preservation is cancer. Oncofertility, a field at the intersection of oncology and reproductive medicine, addresses the fertility needs of cancer patients scheduled to undergo gonadotoxic (toxic to the reproductive organs) treatments such as chemotherapy, radiotherapy, or radical surgeries. In women, ovarian stimulation followed by egg or embryo cryopreservation, is the standard approach before starting cancer treatment.
Emerging options such as ovarian tissue cryopreservation show promise, particularly for girls prior to puberty, or women who may not be able to undergo traditional egg retrieval. This experimental technique involves removing and freezing ovarian tissue, which can be transplanted back later, potentially restoring fertility. There have been reports of re-implantation leading to successful full-term pregnancy. In men, sperm banking is a well-established technique, while testicular tissue freezing is an emerging alternative for boys prior to puberty.
Beyond oncology, fertility preservation is gaining traction among individuals seeking to delay childbearing for social or medical reasons. The trend of “social freezing” is rising, where women opt to freeze their eggs in their late 20s or early 30s to manage the natural decline in ovarian reserve over time. Men are also freezing their sperm for similar reasons. It is imperative that fertility specialists provide comprehensive counselling to help patients make informed decisions aligned with their values and circumstances. Moreover, success is not guaranteed, and patients must be counselled on realistic expectations. Additionally, ethical considerations, including the disposal of unused eggs, sperm or embryos, and the psychological impact of postponing reproduction, require care.
Despite its benefits, fertility preservation faces challenges. Accessibility remains a major issue, particularly in low-resource settings where high costs and limited infrastructure restrict availability. The cost of fertility preservation can be a barrier for some patients. While health insurance policies vary, many do not cover these procedures, leading to financial strain. Advocacy is needed for more inclusive policies that support individuals seeking to preserve their reproductive health.
