You may be curious about the names of fertility drugs or medications you hear or read about that are used in fertility treatments and wonder which ones are the best. The word ‘best’ is fully dependent on each individual’s diagnosis, medical history, and treatment plan. There is usually no ‘one medication works for all’ solution.

You may have heard of brand names such as Clomid (Clomiphene), Femara (Letrozole), Gonal-F (recombinant follicle-stimulating hormone – rFSH), and Menopur (urinary follicle-stimulating hormone – uFSH). These are all fertility medications, but they differ in how they work, how they are administered, and which situations they are best suited for. Clomid and Femara are oral medications often used as first-line treatments for inducing ovulation, while Gonal-F and Menopur are injectable hormones used for more advanced treatment, such as in vitro fertilisation (IVF).

Clomid (Clomiphene citrate)

  • Mechanism: An oral medication that stimulates the release of follicle-stimulating hormone (FSH) and luteinising hormone (LH) from the pituitary gland, promoting the growth and release of eggs from the ovaries.
  • Uses: Often the first-line treatment for infertility caused by irregular or absent ovulation, including in women with polycystic ovary syndrome (PCOS). It can be used under the guidance of a fertility expert for timed intercourse or intrauterine insemination (IUI).
  • Side effects: May cause hot flushes, mood swings, headaches, and visual disturbances. It can also thin the lining of the uterus, which may affect embryo implantation.
  • Risks: Carries a higher risk of multiple pregnancies (5–10%) compared to Femara.

Femara (Letrozole)

  • Mechanism: An oral medication that temporarily lowers oestrogen levels, stimulating increased FSH production by the pituitary gland. Its effects are shorter-lasting than those of Clomid.
  • Uses: Commonly used to stimulate ovulation, particularly in women with PCOS, especially those who are medically overweight. Often prescribed for patients who do not respond to Clomid. It can also be used with timed intercourse or IUI.
  • Side effects: Generally associated with fewer side effects than Clomid and does not negatively affect the uterus lining or cervical mucus. Possible side effects include fatigue, headaches, and hot flushes.
  • Risks: Lower risk of multiple pregnancies compared to Clomid and injectable gonadotropins.

Gonal-F and Menopur (types of FSH known as gonadotropins)

  • Mechanism: Injectable hormone medications.
  • Gonal-F: A recombinant form of FSH (follitropin alfa) that directly stimulates the ovaries to produce multiple egg follicles.
  • Menopur: Contains both FSH and LH activity sourced from the urine of postmenopausal women (menotropins), stimulating the ovaries and aiding egg maturation.
  • Uses: Typically used for more advanced fertility treatments like IVF or ovulation induction in women who have not responded to oral medications. These provide more direct and stronger stimulation of the ovaries.
  • Side effects: More frequent and severe compared to oral drugs. Common side effects include injection-site reactions, headaches, and abdominal pain.
  • Risks: A major risk is Ovarian Hyperstimulation Syndrome (OHSS), a condition where the ovaries become swollen and painful. This is closely monitored by the fertility team prescribing the medication. The risk of multiple pregnancies is also higher than with oral medications.
