How Thalassemia Affects Fertility in Men and Women

Thalassemia is an inherited blood disorder that can affect reproductive health, fertility, and pregnancy outcomes in both men and women. Thalassemia is a genetic condition that affects the body’s ability to produce healthy haemoglobin, the protein in red blood cells responsible for carrying oxygen. Reduced haemoglobin production leads to anaemia, fatigue, and other health complications. The disorder is inherited from one or both parents and is most common in people of Mediterranean, Middle Eastern, Asian, and African backgrounds. There are two main forms of Thalassemia: Alpha Thalassemia and Beta Thalassemia. Both types can range from mild states (Thalassemia Minor) to severe disease (Thalassemia Major), the latter requiring lifelong treatment and blood transfusions.

Common symptoms of Thalassemia include chronic fatigue and weakness, pale or yellowish skin, shortness of breath, dizziness, delayed growth and puberty and persistent anaemia. People with Thalassemia Major often require regular blood transfusions, which can lead to iron overload in the body. Excess iron may accumulate in endocrine organs such as the pituitary gland, ovaries, and testes, disrupting hormone production and reproductive function.

Impact on fertility

In women, Thalassemia may contribute to irregular menstrual cycles, delayed puberty, ovulation problems, hormonal imbalances, early decline in their egg reserves (premature ovarian ageing), difficulty conceiving and increased pregnancy complications, including pre-eclampsia, gestational diabetes, restricted foetal growth, and preterm labour. Up to 60% of men with Thalassemia Major experience impaired reproductive capacity due to combined hormonal and tissue damage. In men, iron overload can reduce testosterone levels and cause testicular damage due to excess iron deposits directly into the testicular tissue, causing cellular damage and reducing sperm production, potentially leading to infertility.

Fertility interventions

Despite these challenges, advances in fertility care and pregnancy management mean many individuals with Thalassemia can still conceive and have healthy pregnancies with appropriate treatment and monitoring. This includes regular blood tests and monitoring, iron chelation therapy to reduce iron overload, hormone assessments and therapy, fertility evaluations and specialist reproductive care.

Genetic counselling is strongly recommended before conception. Since Thalassemia is inherited, testing both partners can help determine the risk of passing the condition to children. Couples may explore reproductive options such as IVF, embryo screening, donor gametes, or prenatal genetic testing.
